If you would like your eligible child to receive the vaccine, please do the following:
1. REGISTER YOUR CHILD FOR AN APPOINTMENT
You must access one of the links below for the day that you are signing up for, and complete the online form for one of the available appointment times. Please only sign up for one time slot for each child.

2. READ AND SIGN PARENTAL CONSENT
You can sign the consent in two ways: 1) You can complete it as part of the online form when you sign up for the appointment (link above); or 2) You can complete the paper version, which is attached to this correspondence.
By signing the parental consent form, a parent is not required to be present during these vaccination clinics being held at the school; however, parents are welcome to accompany their child during their appointment time.
3. ENSURE YOUR CHILD HAS TRANSPORTATION
If your child attends school in person, and will attend school on the day of their scheduled appointment, they will be transported as usual (i.e. school bus or parent drop-off/pick-up). If your child is virtual, or if your child is not present for in-person learning that day, transportation will need to be provided by the parent to and from the school.
We are excited for this opportunity, and hope to see many of you there. Please remember, while vaccinations are encouraged by public health officials to help reduce the spread of COVID-19, there are currently no requirements by HPCS for students to have the COVID-19 vaccine- it is a parent decision.
